Depending on the surgeon of choice as each technique when doing the famous traditional facelift is quite different and varies from patient to patient. I ask this question and many others on realselfdotcom where you can ask medical questions. So let’s get started on a traditional facelift.

Q. What is a traditional facelift?
A. Hi, non smiling photos of your face from the front and side would help in the evaluation.  I have performed facelifts for over 30 years and have performed many minimally, invasive type facelifts.  Traditional facelifts have long incisions that begin in the temporal hairline, around the entire front of the ear, inside the crease behind the ear and then continue inside the posterior hairline.  This type of facelift is very invasive with extensive tissue undermining that has a recovery time of months.  I have performed many of these facelifts however in today’s busy lifestyle, very few seem interested in this extensive recovery, bruising, swelling and downtime.  The vast majority, of my facelift patients choose the minimally, invasive SMAS type facelift as described below.

Following the beauty principles outlined in my book on face and body beauty, women look the most feminine, youthful and attractive with heart shaped faces.  Heart shaped faces have cheeks that are full and round in the front.  Cheek augmentation with a dermal filler or using cheek implants for a permanent enhancement will create full, round cheeks that will feminize the entire face.

If the chin is weak, this creates an imbalance making the nose appear larger, the mid face top heavy and the lower face look short that de-emphasizes the lips and allows early formation of a double chin.  Chin augmentation using a chin implant will add projection to the chin creating harmony and balance to the lower face.   I have found that placement of a silastic chin implant, through a small curved incision under the chin (also allows excess skin removal) to be very safe, quick and highly effective.

If you have “jowls” these are sagging facial tissues and an indication for some form of a SMAS facelift.  The underlying SMAS layer, of the face, must be dissected, lifted, trimmed and re-sutured (not merely folded or suspended with threads or sutures that will not last).  The excess skin is then removed and the facelift incisions closed. My most popular facelift is the minimally invasive, short incision facelift that has all the benefits of more invasive facelifts (traditional, mid-face, deep plane, cheek lift and subperiosteal facelifts) but with these added benefits:

  • very small incisions
  • minimal tissue dissection = less bruising and swelling = rapid recovery
  • can be performed in 90 minutes or less, with or without general anesthesia
  • no incisions within the hair = no hair loss
  • excess fat can be removed
  • excess skin removed
  • cheeks, chin and jaw line can be augmented with dermal fillers (I prefer Restylane Lyft) or facial implants
  • most patients fly back home to parts all over the world in as little as 3 days post-op

I combine facial shaping with every facelift procedure.  When jowls are present, these should be done in concert and not alone or separately in order to create a naturally, more attractive face.

Q. What is the recovery time for a traditional facelift?
A. Your instructions will vary from surgeon to surgeon, so it’s important to discuss your surgeon’s specific instructions to maximize your recovery.  I generally tell patients that they must avoid turning the head or stretching the neck for 2 weeks. When you turn, move the shoulders and head as one unit or as though you had a “stiff neck.” This is important to prevent stretching the healing incisions and causing the scars to widen, as well as to prevent putting tension on the deep sutures beneath the skin, which could cause them to pull loose.  Because of this risk, we advise patients to not drive for 2 weeks following surgery. In order to minimize swelling, you are advised to sleep with the head of your bed elevated or in a recliner for 2-4 weeks post-op.  With some make-up, your hair worn down, a neck scarf, and thick-rimmed sunglasses, you should be able to go out in public within a few days with minimal attention drawn to yourself. Most patients take 2 weeks off from work after a rhytidectomy and are able to resume their exercise regimen within 3-4 weeks.

Q. How soon can a person return to normal activity after a traditional facelift?
A. Recovery following a facelift can vary, and the level of recommended activity will also vary with each surgeon.  Typically, it will take 1-2 weeks for the immediate bruising and swelling to resolve although subtle changes will continue for several weeks to months.  I advise my patient that they should be able to drive within 1-2 weeks and can return to a sedentary job by 10-14 days.  However, it will be at least 3 weeks before one can resume strenuous activity like working out or heavy lifting.  I would suggest that you speak with your surgeon regarding his or her recommendations for your recovery because it may be slightly different based on your individual needs.

Q. What are the pros and cons of a liquid facelift?
A. The face ages in three ways: Sags and bags, Lines and Hollows, Tone and Texture. When we are young our face is like an inverted triangle, with the fullest part being from the forehead to the bottom of the nose. As we age, the triangle flips upside down with our upper face being the most narrow. A “liquid facelift” is achieved with injectables like Voluma, Radiesse, and Juvederm plus Botox for smoothing lines in the forehead, around the eyes, lip and chin areas.  The first step to determine if a liquid facelift is right for you is evaluating your face to determine where your facial aging in occurring. Often times patient complain of nasal labia folds (the lines that form from the nose to the mouth area), fallen or sunken cheeks, hollowed under eyes and temples, thin lips and weakened jawlines. The key to a liquid facelift is keeping it natural looking and not getting overfilled, therefore technique and the skills of the injector make a significant difference. We use mostly micro-cannulas for our injections, which minimize bruising and allow for smooth contouring of the face. There is an Abraham Maslow saying, “If all you have is a hammer, then everything looks like a nail.” In this case it applies to choosing a plastic surgeon to access your facial aging process and determine a course of action. Often times, a patient is not a good candidate for a liquid facelift and needs to consider an alternative procedure such as a non-incisional Radio Frequency facelift or a traditional facelift. While fillers can be a fantastic solution for staving off the aging process, eventually, you will want to know your options for continuing to look natural as well as youthful.

Breast Augmentation #plasticsurgery

Here is an excerpt of an exclusive interview with Dr. Malcolm Lesavoy on breast augmentation.

Q.What is breast augmentation?
A. Breast Augmentation is for women who have small breast and want to have larger breast. Now in my opinion if you want natural looking breasts you’ve come to the right place. In my hands the surgery itself takes 45 minutes to an hour under general anesthesia. They’re three incisions that are commonly used, one of them is in the crease underneath the breast, the other is an incision around the nipple in the areola area, third is done in the armpit. About 60% of surgeons use the crease underneath the breast, 20-30% do it around the areola and about 5% or so of us which is the method I prefer to use the armpit for the incision because there’s no scar. It’s in the upper crease where you shave. It works out real well. There are two different kinds of implants saline and silicone. Both of which have a silicone shell. One feels like a balloon filled with water which is the saline. While the other feels like a balloon filled with gel which is silicone. 90% of surgeons now use silicone implants. I prefer the silicone implants because there is a down side. There’s a downside to both, but with saline implants they have a tendency to ripple. So if you have thin breasted skin sometimes you can see or feel those ripples, the silicone implants you can’t. There’s no disease aspect for either one because they’re both silicone shell. One is filled with water the other is filled with gel. The main problem with breast augmentation is that scar tissue forms around the implant which basically starts to contract and gets tighter around the implant. For example if you have a balloon that is filled with water and you’re squeezing that balloon with your two hands the part that comes out between your fingers feels firmer. That’s what we call encapsulate contraction. We don’t know what causes it, we don’t know how to prevent it. When it occurs the issue can be fixed it only happens as of lately 2-3% of the time. Now twenty years ago that number was 15% that it would occur. I tell my patients what you’re paying for is my judgement. You can’t estimate a size because it comes in centimeters. I tell my patients you can’t fit a size 5 in a 4 shoe. So we put different sizes in and out until we find the largest size that look natural at the time of surgery.

Q. What is the recovery time for breast augmentation?
A. Normally patients are in the recovery room for 45 minutes to an hour then they go home. The following day they will feel sore like they’ve been doing a ton of push-ups. You can drive in two to three days, and are totally back to normal in 2-3 weeks.

Q. What is coolsculpting, and how does it work?
A. We have been doing Coolsculpting in our clinic for almost two years and our patients probably rate it with over a 90% “happiness” factor.  The key is patient selection for this form of non invasive fat reduction that works by selectively freezing the fat and not the skin.  The procedure is for pinchable fat in patients that have relatively good skin quality. If patients have poor skin quality with stretch marks and stretched out abdominal muscles, then this technique will not improve those issues. The goal is for fat reduction.  It is probably around a 25% fat reduction with one applicator and the process for fat resorption takes a few months.  It is permanent fat reduction but one applicator doesn’t do the job.  It takes a regional treatment plan. But during these treatments patients fall asleep, read their emails or watch a movie.

We have seen some amazing results in realistic patients that are committed.  We also have a few patients that are better surgical candidates but for whatever reason they are unwilling or unable to have surgery. These patients can still have fat reduction but their skin quality is poor.  But they understand the limitations prior to treatment. We are looking into adding a body skin tightening device after treatment with coolsculpting to see if we can improve the result even more. This radiofrequency device may also be to smooth out smaller fatty areas that are not treatable by coolsculpting or may have some ability to improve cellulite!

Q. What are the side effects if any from coolsculpting?
A. Coolsculpting can remove a small area of fat, but can cause bruising, swelling, inflammation that can last a few weeks.  Sometimes indentations can occur if there is no overlapping of treatments.  Sculpsure has less inflammation and no risk of indentations.

Q. Does coolsculpting have a long lasting effect?
A. Yes-once you treat in an area, CoolSculpting removes the fat in the area for good. Keep in mind that it may take 2-3 treatments to achieve the desired results. This type of procedure is meant to coincide with a healthy diet and exercise routine. Should you gain weight it will be gained in areas that have not been treated with CoolSculpting.

We had wonderful opportunities to meet one on one with brands. I personally loved that major brands took time to give us feedback on what works and what doesn’t. I was told that my site needed a new look so the brand pro recommended that I spend a little money on my template. She also suggested that I put more ad space opportunites to earn extra income. Be sure to know your alexa rating. It comes in handy when speaking to brands about your numbers.

This boutique conference offered us the best of blogging and networking. Day two break out sessions were a plus for me in that you could choose which session best fits your needs. Did I mention winning a Jord wood watch? Soo excited about it. I can not wait to see what’s in store for next year. Didn’t get to join us this year? Thursday’s VIP reception was a girl’s paradise and its networking potential was priceless. Where woman didn’t apologize for being who they are and wore the smack out of makeup. Her style gives you a spirit to dress up every day with grace and elegance in mind. Though I don’t wear makeup her product makes you want to try it daily and leave you to feel like a Hollywood dame.

The day of the conference was stellar with guest appearance by fashion extraordinaire Daniel Musto his inspiring speech and lovely hashtag neck wear left us with a notion that everyone matters and to always smile when you meet someone. We went on to experience on how to network and the tech side of blogging. Coffee was provided by Stumptown Coffee Roasters Lunch Stone Fire Grill Drinks by OnBeverages.

The conference offered us the best. Second day of the conference I started off with Stumptown coffee which by the way keeps you alert throughout the day. Our guest speaker for the morning was none other than Patricia Velasquez whose words of wisdom I will cherish. “Life will never give you something you can’t handle, everything that happens is good for you, no matter how bad it looks.” Legal one on one with Rosa Elena Sahagun was a must for getting your contracts in writing even the verbal ones. She was  followed by SEO expert Claudia Havi Goffan whose positive energy kept the room lit with sunshine on it’s brightest day.

Fournier provided great tax advice. “Put aside 25-45% thru the year to pay taxes at end of year or quarterly. Percent flexuates with income.” After taxes came lunch provided by Fresh Brothers. I couldn’t get a proper photo because my face was in the plate. lol I met so many business owners who are willing to take the next step to optimizing their visibility. We had a pretzel chips snack break followed by an ethics panel and an inspiring story of business owner Ana Guajardo of ChaCha covers for your nails.


I would recommend this event to every blogger or business owner serious about taking steps to enhancing or even to give their blog a face-lift for the globe to see. Personally for me the experience was a life lesson that the universe will give you everything you need. All you have to do is show up and work hard. A special thank you to the organizers of the event for such an amazing experience. I love seeing woman grow together, its very rare. Thank you for your wisdom, knowledge and understanding of how a business should run. See you next year!!!! #lbnc15
