Dr. Jaime S. Schwartz MD, FACS, is board certified in plastic surgery. He earned his bachelor’s degree in cellular and molecular biology from Tulane University and went on to attend Georgetown University School of Medicine. He is a member of the American Society of Plastic Surgeons (ASPS), and a diplomate of the American Board of Plastic Surgery. Most recently he completed a Plastic and Reconstructive Surgery residency as well as a Plastic and Reconstructive Surgery Post-graduate Fellowship at the University of Miami/Jackson Memorial Medical Center Program. In our exclusive interview we dive into many areas of plastic surgery he loves and why he chose the profession.

Q. How did you know that plastic surgery was right for you?
A. I knew I wanted to enter the field of medicine as a surgeon, because I have great hand eye coordination and felt that this area would best match my skills.  I decided Plastic Surgery was a perfect fit because I love to see immediate results and because it’s a joyful sector in the medical field…I can help someone gain more self confidence in their body and I can experience their joy when they see their results.

Q. What is a traditional facelift? How do you perform it? How soon can one return to regular activity after the procedure?
A. There are several different variations of face lifts, differing by size, placement, and amount of incisions; procedure; and the results. Depending on the degree of change you’d like to see, your facelift choices include a traditional facelift, limited incision facelift or a neck lift. Most incisions are made in front of and behind the ear, placed where it will fall into the natural crease of the skin. On occasion, the incision will extend into the scalp. Face lifts for males will have the incisions fall into the natural hair lines.

A traditional facelift, or rhytidectomy, is a surgical procedure that improves visible signs of aging in the face and neck, such as: Relaxation of the skin of the face causing sagging, deepening of the fold lines between the nose and corner of the mouth, fat that has fallen or has disappeared, jowls and loose skin and excess fat of the neck that can appear as a double chin.

In a traditional facelift, incision often begins in the hairline at the temples, continues around the ear and ends in the lower scalp. Fat may be sculpted or redistributed from the face, jowls and neck and underlying tissue is repositioned, commonly the deeper layers of the face and the muscles are also lifted. Skin is redraped over the uplifted contours and excess skin is trimmed away.  A second incision under the chin may be necessary to further improve an aging neck.

RECOVERY
Medication will be prescribed for the recovery period, as mild discomfort and moderate swelling of the eyelids and other facial areas may occur. After 2 weeks, the swelling will subside enough to allow some social activity. The typical recovery period is 4 weeks; most surgeons will recommend a facelift to be done a minimum of 3 months before any major event.  It is very important to note that smoking severely compromises blood flow to the skin, which will adversely affect the healing process.  Smoking, Herbal supplements and alcohol should all be avoided at a least four months prior and post-surgery.

Q. What is breast augmentation? How do you perform it? What’s the recovery time?
A. Breast augmentation can give women with small or unevenly sized breasts a fuller, firmer, better-proportioned look through the placement of implants in the breast. Women may elect to undergo breast augmentation for many different medical and aesthetic motivations, including balancing breast size and compensating for reduced breast mass after pregnancy or surgery.

The implant placement refers to where the implant is positioned..under the Pectoralis Major (chest) muscle or over it, which places the implant only underneath the breast tissue. In almost all cases I place the implants under the muscle. The breasts feels much softer and look more natural due to a layer of “buffer” (the muscle) covering the implant. Other significant benefits include: the long run opposing force of the muscle against the implant keeps the implant from stretching out the breast skin and a significantly less chance of capsular contracture (less chance implants get hard). Sub-muscular placement creates more tenderness for a short period of time post-surgery, but is worth it.  I also utilize the latest techniques resulting in minimal or little scarring. The most common incisions include an inframammary incision (near the crease under the breast), an axillary incision (in the underarm area), or a periareolar incision placed at the edge of the areola (the pigmented skin surrounding the nipple). Incision patterns vary based on the type of implant, degree of enlargement desired, patient anatomy, and patient preference.

Augmentation recovery time is approximately two weeks. Immediately after surgery, during your breast augmentation recovery, you will be taken into a recovery area for close monitoring. Your breasts will be wrapped in gauze dressings and an elastic bandage or support bra will minimize swelling and support the breasts as they heal. You are typically discharged after an hour.

We typically prescribe postoperative medications in advance. Medications may include pain pills and an antibiotic to prevent infection. Because it is possible to bleed into the pockets around the breast implants for the first several days, take it easy until you have permission to increase your activity level. Acute pain typically subsides after one to five days, but you may experience soreness and swelling for a few weeks.

Q. Do you prefer silicone, saline or fat transfer?
A. For breast Augmentation, I prefer Silicone or Saline Implants.  The choice of each one really depends on the patient’s unique goals and budget.

Q. Who is the right candidate for this procedure?
A. The right candidate is any adult who wants to enlarge a naturally small breast, restore breast volume loss as a result of breast-feeding, or due to weight loss, and/or achieve better symmetry when breasts are moderately disproportionate in size and shape.  We also conduct Breast Augmentation on Males who wish to go through Transgender Reassignment.

Q. What does the mommy makeover tummy tuck includes? How is it performed and what is the recovery time? What kind of liposcuplturing is involved?
A. A mommy makeover is a perfect way for patients to make big strides in their post-baby figure in a short amount of time. Since a combination of procedures are performed at once, patients will only have to go through one recovery period and surgery. After the procedure, patients can expect to have some pain and swelling in the first few days following surgery. After 2-3 weeks, the body will still be recovering, but patients are able to resume activities, so long as they aren’t overexerting themselves. The healing process in total is approximately 6 weeks (healing is very unique to each person) and you can enjoy a beautiful body, alongside beautiful children.

In my practice, all patients are handled on a case by case basis.. During the initial consultation, the procedure and recovery process will be discussed in detail as it pertains to each unique situation. Some common surgical combinations include:

  • Breast Lift and Tummy Tuck
  • Breast Augmentation and Tummy Tuck
  • Breast Lift and Liposuction
  • Breast Augmentation and Liposuction
  • Liposuction and Tummy Tuck
  • Breast Reduction and Liposuction

How it works
Tummy tuck can be performed on an outpatient basis under general anesthetic. There are several different types of tummy tucks, but all of them involve the removal of excess skin and fat using a lower abdominal incision. The abdominal muscles are tightened and the remaining skin is then pulled downward. In most cases the belly button must be repositioned resulting in a circular scar around it. The lower abdominal incision usually extends from hip to hip and is discreetly concealed below the bikini line, so you can still hit the beach. During a tummy tuck, drains are used to assist healing, and they’re removed 1 to 3 weeks after the surgery.

Liposculpture
Liposuction is performed on an outpatient basis under general anesthetic. Power-assisted liposuction utilizes a vibrating cannula that is inserted through tiny incisions to gently shake the fat cells loose. This facilitates suction removal of fat with more precision and less trauma than other liposuction techniques.  During Liposculpture we reprocess the fat to supplement other parts of the body (i.e. the buttock) to create a more voluptuous look.

Q. Fat transfer is popular in regards to the Brazilian buttocks lift. How do you perform it and what is its recovery time? Do you use implants or fat transfer? Which option is more practical?
A. Absolutely…Brazilian Buttock Lifts is becoming one of our most popular procedures.  It involves fat transfer that takes fat cells from unwanted areas, and repurposes these cells to give you a beautiful curved figure, exactly where you want it. If you’re looking to reshape your body with a defining, formed rear-end, then a Brazilian butt-lift is a very safe and effective option with reasonable recovery time.

Recovery|
The recovery process may change from person to person, and that is why I handle each patient with unique care and attention. However, you can expect to fully experience your beautiful new curves within a few weeks. Here is what a standard recovery period may look like.
First week – Potentially significant swelling and bruising, therefore, patients may choose to avoid others in a social or professional setting
Second week – Bruising will begin to subside, but patients will still experience swelling.
Third week – The overall appearance and swelling will improve greatly. Patients can typically resume their normal, day-to-day activities.
Fourth week – By this time, the swelling will have subsided enough for you to begin to appreciate your beautiful new curved shape. However, some swelling may persist for a few more weeks. During your initial consultation, I will discuss the full process in detail as it pertains to your unique situation. Because the surgery uses fat from your own body, it is often considered one of the most natural ways to add shape and definition to your buttocks, so I prefer it over an implant.

Here is an excerpt of an exclusive interview with Dr. Malcolm Lesavoy on breast augmentation.

Q.What is breast augmentation?
A. Breast Augmentation is for women who have small breast and want to have larger breast. Now in my opinion if you want natural looking breasts you’ve come to the right place. In my hands the surgery itself takes 45 minutes to an hour under general anesthesia. They’re three incisions that are commonly used, one of them is in the crease underneath the breast, the other is an incision around the nipple in the areola area, third is done in the armpit. About 60% of surgeons use the crease underneath the breast, 20-30% do it around the areola and about 5% or so of us which is the method I prefer to use the armpit for the incision because there’s no scar. It’s in the upper crease where you shave. It works out real well. There are two different kinds of implants saline and silicone. Both of which have a silicone shell. One feels like a balloon filled with water which is the saline. While the other feels like a balloon filled with gel which is silicone. 90% of surgeons now use silicone implants. I prefer the silicone implants because there is a down side. There’s a downside to both, but with saline implants they have a tendency to ripple. So if you have thin breasted skin sometimes you can see or feel those ripples, the silicone implants you can’t. There’s no disease aspect for either one because they’re both silicone shell. One is filled with water the other is filled with gel. The main problem with breast augmentation is that scar tissue forms around the implant which basically starts to contract and gets tighter around the implant. For example if you have a balloon that is filled with water and you’re squeezing that balloon with your two hands the part that comes out between your fingers feels firmer. That’s what we call encapsulate contraction. We don’t know what causes it, we don’t know how to prevent it. When it occurs the issue can be fixed it only happens as of lately 2-3% of the time. Now twenty years ago that number was 15% that it would occur. I tell my patients what you’re paying for is my judgement. You can’t estimate a size because it comes in centimeters. I tell my patients you can’t fit a size 5 in a 4 shoe. So we put different sizes in and out until we find the largest size that look natural at the time of surgery.

Q. What is the recovery time for breast augmentation?
A. Normally patients are in the recovery room for 45 minutes to an hour then they go home. The following day they will feel sore like they’ve been doing a ton of push-ups. You can drive in two to three days, and are totally back to normal in 2-3 weeks.

Dr. Richard Ellenbogen is a board certified plastic surgeon. He received his training from the University of Florida, University of Miami Leonard School of Medicine, The Prenatal Determination of Fetal Sex Using Maternal Blood Through Chromosome Analysis Research Medical School & Albert Einstein College of Medicine respectfully. I had the privilege of an exclusive interview in his Los Angeles office.

So you say you want a youthful look, fuller lips, no frown lines, no marionette mouth. You may be a candidate for fat grafting. Slam on your breaks and get ready because every procedure that I have found has everything to do with fat transfer, fat grafting or fat injection. Dr. Ellenbogen  rediscovered the technique in the early 80s where research was conducted  on a few people to see if this procedure would enhance the job. He was successful in his findings, the technique has been used by most physicians all over the world.

Q.What is Fat Grafting?
A. It is transferring fat from areas in which you have excess fat, such as the outer thighs or the tummy area, and injects it into areas that may be lacking in volume, such as the face, hands, breasts or buttocks.

Q. Who is the best candidate for Facial Fat Grafting?
A. Fat grafting is for someone who wants to add volume to their cheeks, lips deep wrinkles, nasolabial folds and under the eyes. We like to perform this surgery with the rhinoplasty which is my specialty though its difficult to add fat to the nose.

Fat grafting is a minimal non-invasive procedure with no downtime. During our interview I had the pleasure of meeting one of Dr. Ellenbogen’s patients that had fat grafting done. Fat was removed from her love handle and injected into her lower eyes, lips and marionette lines for more volume to the face. Having had the non-invasive surgery 24 hours from the time I saw her was remarkable. It really just looked like she had on makeup with very little swelling and little to no bruising I was impressed.

#funfact Dr. Ellenbogen is a portrait painter. His approach to the rhinoplasty procedure is like no other. He has a knack for picking up, what a patient wants to achieve.

Other procedures that use fat injections are breast augmentation, brazilian butt augmentation and hand surgery.

Breast augmentation we know uses saline or silicone implants also applies the fat grafting technique in persons that have smaller sized breasts that may just want to go up a cup size. Depending on size and proportion of the butt. The brazilian butt augmentation is where the fat is transferred from an area that the patient desired that the fat should come from is removed from the love handles, or thighs and is injected into the butt for a more curvier look. It all depends on if the patient has the fat to use. Hand surgery is quite different in that the aging process causes it to look thin. Fat transfer makes the hand look more define.

On to more charitable note Dr. Ellenbogen is helping to raise funds for Hospital de la fe in San Miguel de Allende, Mexico and Project Happy Face. The goal to help children and adults who have congenital or injury induced deformities. Hospital de la fe is not equipped currently for these types of procedures so patients are sent to Celaya Clinic of Dr. Valazquez and Dr. Berlanga for dental and speech respectfully. Dr. Malcolm Lesavoy is a board certified plastic surgeon. He has worked as a professor at UCLA Medical Center for over 25 years and Chief of Plastic Surgery at Harbor UCLA Medical Center. He started the whole plastic surgery division at City of Hope for reconstructive plastic surgery. Dr. Lesavoy is an author of three books and has published over 100 peer-reviewed papers. You may recognize him from the hit t.v. series Extreme Makeover. Read on in our exclusive interview….

Q.How does the public know whether a doctor is a real plastic surgeon or not?
A. The training of a plastic surgeon is the longest training of any specialty in all of medicine. They graduate high school of course, four years of undergraduate school then 4 years of medical school which they will earn an MD, they go on to 7 or 8 more years of residency specialty training first in general surgery then plastic surgery to then become a plastic surgeon. Yet, they’re still not board certified. They then take an oral examination one year after they finish their residency in plastic surgery and if they pass which 40% of doctors do. They go on to then wait another year for an oral exam which only 25% pass. They then become a board certified plastic surgeon. Most surgeons don’t really start their own private practice until about 34 or 35 years of age. The only way to tell if a doctor is board certified is for an individual to contact the American Society of Plastic Surgeons.

#Fact If you look in the yellow pages under plastic surgery 60% of them are not plastic surgeons.

Q. Dr. Lesavoy explained to us your experience on Extreme Makeover.
A. I did the first five extreme makeover transformations. I only did them for one reason or one purpose. To explain to the public what a real plastic surgeon does. So the producers on the show assured me that they would have the number at the bottom of the screen so the public can find out if the surgeon is board certified through the American Society of Plastic Surgeons. I stop doing the show and referred it to other Plastic Surgeons.

Q. Tell us more about the Lesavoy Luxury Lift.
A. Some surgeons are doing face-lifts under local anesthesia. Its a tremendous boom, its less cost. You don’t have to use a major operating room. It includes face, neck, upper and lower eyelids. What I do in my office, we have a minor operating room here. The only thing a patient feels is a couple of shots. The patient is then rapped up and we see them 24 hours later and the recovery would be 2 weeks.

Q. Do you have any favorite charities?
A. I use surgery for charity. I go overseas to Thailand and Vietnam. Smile train is one that I donate my time to. I go on these trips in order to teach other surgeons how to do surgery.

What is your specialty?

We do all types of cosmetic and reconstructive plastic surgery. We offer our clients a variety of services, breast, body, face, liposuction, laser, botox, fillers and more. He is among  a rare few that have successfully reattached a limb and an expert in hand and micronuerovascular surgery.

Can you tell us the difference between botox and fillers?

Botox is a neurotoxin with blocks the action of nerves on muscles.  We use it to target muscles that form wrinkles.  It relaxes the muscles, softening the wrinkle.  It lasts for 3-6 months.  Fillers are injected into the skin to fill in wrinkles, directly softening them.

How do I pick a breast implant?

I prefer the gel implant. “Saline is like a pinto where Silicone is more natural.”  It also depends on the person’s frame, loose skin on the breast and the desired size the patient wants to meet.

What are the risks or negative effects of smoking?

It clogs up the micro-circulation which can cause scaring and infection.

What are your efforts in helping those whom can’t afford the services but need the surgery?

I have an non-profit organization called RESTORE founded in 2008 which provides  resources, hope, motivation and encouragement in restoring lives through reconstructive surgery in third world countries to battered women and children. He also provides education for under privileged individuals in the U.S., about post reconstructive surgery options that are available after breast cancer treatment.

What would you like future clients to know about you?
